Registration

To initiate the registration process you must return the registration form along with a non-refundable $40 registration fee (The registration fee includes a Mascoma Cooperative Preschool t-shirt that is used on field trips and will be given to your child at orientation before the first day of school). The registration fee will only be refunded if the program you have signed up for is full and no spot becomes available by May 1st. You may download (PDF file) and review the registration form.

The following forms (provided to you after receipt of registration form and fee) must be completed and returned to the Registrar along with the deposit in order to secure your child's spot. The deposit is one month's tuition, $150 for Caterpillars and $209 for Butterflies, and covers tuition for May. The required forms are:

• Child Health Form: The State of New Hampshire requires this form to be filled out by your child's physician and have listed immunization dates. This will need to be updated annually. You will be notified prior to the expiration of your child's health form and provided a new form to be completed by your child's physician.
• Child Care Registration and Emergency Information Form
• Handbook Review Form
• Criminal Check Form: Parents may be required to complete a criminal check form
• Child Profile Sheet: This is for the teacher to familiarize herself with the child
• Confidentiality Form
• Website and Photo Permission Form
• Confirmation Form and one month's tuition for deposit ($150 for Caterpillars and $209 for Butterflies)
